Both Preferred Provider Organizations and Health Maintenance Organizations (PPOs and HMOs) will provide you with a list of the doctors, clinics, hospitals and alternative medical facilities that are members of their provider networks.

Just ask your HMO, PPO or insurance carrier for a copy of their health provider list for your area. You can then choose your primary care provider from the list.
